Survery of Services/Products Provided by UW-Madison Departments and Organizations to Other UW-Madison Departments

Please complete and submit the following survey questions. This information will be used by Business Services to compile a categorical list of services and products which can be provided internally (i.e. within the University of Wisconsin-Madison, from one department to another) by University Departments.

Services and products can be fee or non-fee based. Please submit each service/product only once per department. Departments with multiple services or products should submit the form for each major service or product they would like listed.

Services/products submitted for listing must provide a link for an informational web page. Any contact information submitted should not be specific to an individual. A contact phone number or email address must be for a general departmental number or email account, e.g. purch@bussvc.wisc.edu.

Since a central UW area for learning opportunities already exists, submissions for "training" will not be accepted. We will instead refer users to http://www.myprofdev.wisc.edu/.
