Surplus Property Pick-Up
Request Form (Electronic version). This form is used to submit an on-line request
to SWAP for pick-up of surplus property. Please note that SWAP
picks up items only from UW-Madison departments and Madison area
state agencies.
Surplus Property Pick-Up Request Form
(Word version). This is similar to the form listed above, and
is used to provide a printed request. The department should send
the original to SWAP and retain a copy.
