International Health Insurance
Background: The Board of Regents has mandated that all University of Wisconsin students studying/traveling abroad under a UW sponsored program must enroll in health insurance through an insurance plan offered by Cultural Insurance Services International (CISI). The University has worked with this company for many years, and is pleased with their performance and the continuous enhancements they have made to this coverage.
This program is also
available to faculty and staff who are involved in educational programs
abroad on a voluntary basis. This provides an excellent option for staff that are not sure how their personal insurance will respond abroad, or who don't want to risk paying for medical care and submitting for reimbursement after their return. CISI will make direct payments to providers. Individuals should contact their primary health insurer with questions regarding how the coverage works while in travel status. Remember that employees injured in the course and scope of their employment may be eligible for benefits under Worker's Compensation, regardless
of the location of the worksite.

Enrollment: The policy year runs May 1 through April 30. CISI coverage provides primary health protection, with no deductible; medical evacuation and repatriation. Consult the brochure for specific coverage details.

Effective May 1, the monthly premium is $34.00, per month. The plan inlcudes three short term program rate options (One Week Rate, Two Week Rate and Three Week Rate).
The One Week Rate is for a short term program of 1-7 days and is $9.00.
The Two Week Rate is for a short term program of 8-14 days and is $17.00.
The Three Week Rate is for a short term program of 15-21 days and is $26.00.
For programs of greater than 21 days please use the Montly Rate times the number of months needed (i.e. August 1, 2013 to December 13, 2013 would be 5 months times the Monthly Rate of $34).
PROGRAM ENROLLMENT: Program
Offices will need the following information in order to complete
the enrollment: program name, destination,
full name including middle initial, start date, stop date, # months
of coverage, date of birth, and gender. Months are defined as 30
days. Partial months are counted as a full month's charge unless 21 days or less see above rates.
This is a link to an excel file which can be modified for use by each program office to reflect the individual program office or program name, etc. Once completed for an individual program, the reports must be filed electronically with Debbie Beich, along with an e-mail note including the funding string (i.e. fund/account/dept id, prog and project/grant) for that program. There should be a separate report for each program and funding source. Once we receive the report, we will forward the enrollment electronically to CISI and charge the premium accordingly.
Departments should include the premium in the program fee for large group programs. Individuals may enroll directly through this office, if not part of a larger program group. In all cases, the premium is due, in full, prior to departure from the US.
Individual Enrollment: If you are not part of a formal group which has collected premiums as part of your programming fees, or if you are traveling on your own for study abroad, you should complete the Enrollment form and forward it, along with your check, payable to the UW Board of Regents.
Likewise, if you are a faculty or staff member traveling abroad for educational pursuits, you are eligible to enroll using the same form. If you deliver the form to Risk Management at 21 N. Park Street, Suite 6101, you can pick up your enrollment card; otherwise it will be mailed to you.
PLEASE NOTE: WE CAN NO LONGER ACCEPT CASH. PAYMENT MUST BE MADE WITH CHECK OR MONEY ORDER.
Dependent Coverage: The plan will now allow for dependents who accompany enrolled participants. Dependent enrollment requests will be sent to CISI by the participant needing the coverage for his/her dependent(s). Simply complete the following form Dependent Enrollment Form PDF, save and then send as an e-mail attachment to: email@example.com. Within two weeks of receipt, CISI will process the dependent enrollment, charging the premiums to the participant's credit card. Personalized materials for the dependents are sent back to the e-mail address provided. Please note that dependent insurance dates cannot exceed participant insurance dates.

Employees in work status while involved in educational pursuits abroad are automatically enrolled in a special medical evacuation and repatriation program through the EUROP ASSISTANCE program. This replaces the MEDEX program. HOWEVER, it is important to note that EUROP ASSISTANCE is NOT health insurance; it provides coverage for medical evacuation and repatriation only.
In addition, we also have coverage for IDENTITY THEFT PROTECTION for business travelers to help defend their identity and recover any lost or stolen documents as well.
