Camps and Clinics Special Medical Insurance
This program provides $1,000 worth of accident coverage on a primary basis (not subject to co-pay) and another $4,000 of excess accident insurance, for youth programs sponsored by campus departments. Coverage is available at a minimal daily/weekly cost. The premium must be included in the enrollment.
To enroll your program in the coverage, prior to the beginning of the program you must forward a copy of your program brochure, along with the name of the coordinator and an approximate number of enrollees. Once the program is completed you must immediately forward a detailed list, by name, of all attendees in each program, along with the funding string to which premiums should be charged. Potential claims should be reported to our office when they occur and we will send out claim forms to the parents, along with instructions for filing a claim.
