| Return Policy: |
A “Confirmation of Reception” form will be in an envelop on the outside of the package. This form must be filled in and one (1) copy faxed to Serva Nordmark at 011494122712286 and one (1) copy faxed to Crescent Chemical at 631-348-0913 within 2 hours after receipt. To ensure the correct temperature during transportation, a temperature indicator will be included in the shipment. If the enzymes were exposed to temperatures higher than 37°C , one of the circles will turn red. Mark the status on the “Confirmation of Reception” form. If any circle on the indicator form has turned red, do not use the enzymes. Serva Nordmark will not accept reclamations regarding proper shipment conditions if they do not receive the “Confirmation of Reception” form within 2 hours of receipt of shipment. |

Product Evaluation Process and Shipping Schedule |

Contractor must ship 2 evaluation sets (2 vials of Collagenase and 4 vials of Neutral Protease) with 13 sets of the same lot of each product to be held in reserve until the department deems the lot suitable for use. If the evaluation sets are deemed acceptable by the department the remaining reserved 13 sets of enzymes will be shipped within 14 days of notification by the department. (Since material must be shipped on dry ice from Germany, shipments are only dispatched on Monday or Tuesday). If the lot is deemed unacceptable by the department, one additional evaluation set of enzymes consisting of new lots (1 vials of Collagenase NB1 and 2 vials of Neutral Protease) will be shipped to the department at 50% off the usual price, with 13 sets of the same lots of each product held in reserve until the department deems the lots suitable for use. To be realistic, in the near future we will not have several lots, so only one additional lot evaluation is possible at a time. Upon selection of a suitable lot the department will be billed for a total of 15 sets of each product. |
