SCOPE: Contract for provide courier service for the pickup of Newborn Screening specimens from designated birthing hospitals, birthing centers, and selected clinics/midwifes in the state of Wisconsin with delivery to the State Laboratory of Hygiene (WSLH), Newborn Screening Laboratory (NBS).

The State Lab of Hygiene is responsible for receiving dried blood specimens from all birthing hospitals and medical centers in the state of Wisconsin for the Newborn Screening program, testing the specimens for a variety of “indicators” and then providing that information back to the hospital, medical facility, or health care provider in a timely manner. It is critical that the specimens are picked up daily, transported, and received the next NBS working day so that testing can be performed without any delays to maintain the specimen quality and viability.

NOTE: The State of Wisconsin, Department of Administration has an existing contract with Spee Dee Delivery that is mandatory for Wisconsin State Agencies. This DOA contract will limit the use of this contract by the State Agencies (i.e. DOT, DOC, DNR, etc.) to the extent that Spee Dee can provide services necessary.

SPECIFICATIONS

1. ADMINISTRATIVE SUPPORT STAFFING

Contractor must provide knowledgeable individuals, acceptable to the UW-Madison, who will provide the following administrative support functions:

* A Contract Administrator to act with full authority on the Contractor’s behalf in all matters pertaining to the UW-Madison.

2. HOLIDAYS

For the purposes of this contract the services are needed for the following State holidays: New Year's Day [January 1], Memorial Day [last Monday in May], July 4, Labor Day (first Monday in September], Thanksgiving Day [fourth Thursday in November], Christmas Day [December 25],

Exception: Should News Year’s Day, July 4, or Christmas Day fall on a Sunday, No pickup is required. Should News Year’s Day, July 4, or Christmas Day fall on a Saturday, hospital pickup would be expected only for those that have routine Saturday pickup.

3. SPECIMEN PACKAGE PICKUP AND DELIVERY: The Contractor must deliver the specimen packages to the WSLH by 7:30 am on the day after hospital/birthing center/clinic/midwife pickup. The exceptions are (1) Specimen packages picked up on Saturday are delivered on Monday. (2) specimen packages picked up the day prior to, or the day of, a holiday are delivered to WSLH on the day after the holiday unless the holiday falls on Saturday in which the specimen packages are delivered on Monday

4. PACKAGING: The Contractor must provide packaging that will hold a minimum of 20 newborn screening specimens to be contained within. This package must be strong enough not to tear during routine handling of the package during transportation to the WSLH. Tyvac is not an acceptable packaging material. The contractor must supply this package to the hospitals upon their request. NOTE: With a rare exception (a large birthing hospital) there should be no more than one specimen package per day per hospital pickup for the WSLH NBS laboratory. Should the Contractor pick up a package destined for the another UW Madison department but not the NBS laboratory, the package charge for this additional package cannot be charged to the WSLH NBS account.

WSLH NBS will not be paying for additional packages, oversized or “overweight” packages delivered under this contract.

5. FUEL SURCHARGE: The University will only accept fuel surcharge requests as a price increase request. Fuel surcharges will only be accepted if the Contractor can show that the price of fuel has increased by 25% or more from the date of contract award and will be negotiated at the time the surcharge is requested.

Any proposed fuel surcharge must clearly be identified, measurable and directly related to a defined fuel index. No fuel surcharge charges will be allowed without proper notice to the Contract Administrator.
