SPECIFICATIONS

Lot 1 - 10 ml Gray Stoppered Glass Vacuum Tubes

  1. Gray Stoppered Glass Vacuum Tubes must be 10 ml.
  2. Gray Stoppered Glass Vacuum Tubes must contain 100 mg. of sodium fluoride and 20 mg. potassium oxalate.
  3. Gray Stoppered Glass Vacuum Tubes must have minimum expiration date of 12 months from receipt by WSLH.
  4. Shipments must be staggered, for the following estimated dates for 2011 with 11,000 tubes in each shipment: 7/1/2011, 9/1/2011 and 11/1/2011.  5,000 tube delivery for December 1, 2011.

Lot 2  - 30 ml Screw Cap Glass Vials

  1. The Screw Cap Glass Vials must be 30 ml.
  2. The Screw Cap Glass Vials must contain 300 mg. of sodium fluoride.
  3. The Screw Cap Glass Vials must include a Teflon liner.

Lot 3A - Postmortem Blood and Urine Specimen Collection Kit

1.   Postmortem Blood and Urine Specimen Collection Kits must include a 60 ml screw cap polyethylene vials for urine specimens.
2.   The Urine specimen 60 ml vial must have a secure lid and be leak-resistant.
3.   The Urine Specimen 60 ml vial should be made of polyethylene.
4.   The kit must include a 30 ml screw cap glass vial containing 300 mg. of sodium fluoride for blood specimens.
5.   The screw cap for the blood specimen 30ml glass vial must include a Teflon liner.
6.   The kit must include two self-sealing, leak proof specimen bags, and contain a liquid absorbing sheet.
7.   The kit must include two specimen seals, one for the urine vial and one for the blood vial.
8.   The kit must be pre-sealed after production and include a “Tamper – Evident” shipping seal.
9.   The address label affixed to the top of  the kit is necessary for shipping and must include Red print on a Blue Label.
10. The kit must include an “EXEMPT HUMAN SPECIMEN” label.
11. The glass vials need to be protected from breakage when shipped.

LOT 3b   - Postmortem Blood Specimen Collection Kit

  1. The kit must include a 30 ml screw top glass vial containing 300 mg. of sodium fluoride.
  2. The screw cap for the 30 ml glass vial must include a Teflon liner.
  3. The kit must include a self-sealing, leak proof specimen bag, and contain a liquid absorbing sheet.
  4. The kit must be pre-sealed after production and include “Tamper – Evident” shipping seal.
