SCOPE: The Wisconsin State Laboratory of Hygiene (WSLH) and Wisconsin Veterinary Diagnostic Laboratory (WVDL) currently have mail picked-up and delivered to and from the US Post Office on Milwaukee Street in Madison. One of the purposes of this contract is to establish pricing for this service specifically, and pricing for general per mile services that may be needed by WSLH and WVDL or any other UW Department on Campus or in the greater Dane County area. Other Departments or locations currently using this contract include University of Wisconsin Press Journals, DOIT Digital Publishing and Printing, Ophthalmology and Visual Sciences, VA Hospital, UW Department of Medicine, UW Department of Medicine CTRI and UW Health Verona Clinic.

A second current use of this contract is the pickup and delivery of water samples taken across the State for delivery to Wisconsin State Laboratory of Hygiene (WSLH) in Madison to be tested and reviewed with resulting reports prepared and shared with other State and Federal Agencies.

SPECIFICATIONS

FOR LOT 1

1. The Wisconsin State Laboratory of Hygiene (WSLH) and the Wisconsin Veterinary Diagnostic Laboratory (WVDL) require the contractor to pick up U.S. Mail (estimated 8-10 BAGS) from the Madison Post Office on Milwaukee Street, back dock, door 26, and deliver to WSLH two (2) sites, 465 Henry Mall, Madison, WI 53706 and 2601 Agriculture Drive, Madison, WI 53718, and the WVDL building at 445 Easterday Lane, Madison, WI 53706. The mail is to be separated for the three sites and placed in containers by the U.S. Post Office.

2. As the Post Office sometimes does not have all the mail on the dock area, the contractor is required to determine that all available mail is there for pick up. This includes going into the sorting area to get the mail not put out on the dock which requires some judgment on the contractor’s employee part to know whether all the mail has been made available. It is important that all available mail is picked up because there are specimens that must be handled within certain time constraints in order to be valid.

HOLIDAY SCHEDULE:

(a) Unless Christmas Eve and New Year’s Eve fall on a Sunday, WSLH will require delivery services.
(b) If the 4th of July falls on a weekend, and depending upon when the 4th of July is celebrated, it might be necessary to schedule deliveries per the holiday schedule.
(c) There will be no mail delivery on Martin Luther King’s Jr. Birthday, which is observed the 3rd Monday in January.

FOT LOT 2 - DELIVERY SERVICES FOR BEACH TESTING SAMPLES
The UW Madison-State Laboratory of Hygiene (WSLH) requires the contactor to pick up shipping containers from 5 different park locations throughout the state and deliver to WSLH, 2601 Agriculture Drive, Madison, WI 53718.

1. Shipping container: Cardboard box around a Styrofoam cooler
2. Size: 11½” W x 10½” H x 9½” D
3. Weight: Empty: 1-1½ lbs; Full: 5-6 lbs
4. Contents: Beach water with ice to keep samples cold (nonhazardous)
5. Container must remain upright
6. Empty containers must be returned to different park locations 2-3 times per week
7. Samples must be pickup at the parks after 12 noon.
8. Samples must be delivered to WSLH by 9 am the next day.
9. At the start of the season the contractor will pick up kits at WSLH on May 18, 2011 and deliver to the State Parks on May 19, 2011 for May 25, 2011 and the week after Memorial Day sampling. There will be 5 kits going to each sampling location.
