Tax Statements

Background: In an effort to increase electronic access to individuals at UW – Madison, tax statement copies are being added to the My UW portal starting the end of March 2007. Paper tax statements will continue to be mailed according to the regulatory guidelines. Access to the tax statements is available online at my.wisc.edu.

What are the tax statements that might be displayed?
There are three types of year-end tax statements produced from the Office of Human Resources:  W-2, 1042-S and Fellowship/Scholarship information letter.  These statements contain information which may have to be reported on your annual tax statements.  For more information on these statements, click on the following web links:

When will my tax statements be posted to the My UW-Madison portal?
Tax statements will be available by the time duplicate statements are made available, which is generally around February 15 of the year following the tax reporting year.

How long will my tax statements be available in the portal?

What if I no longer wish to receive the paper versions of my tax statements?
At this time, there is no provision for making this election.  We are hoping to deliver that functionality at a later date.

How secure is the information displayed in the My UW-Madison portal?
Access to the My UW-Madison portal is secured according to the best practices for web security. The NetID and password are conveyed using Secure Socket Layer (SSL) technology and the password is encrypted before transmission over the Internet. After typing your NetID and your password (which is encrypted on the login page and shows up only as dots or asterisks) at the login page of the My UW-Madison portal my.wisc.edu and hitting 'login', the user is automatically directed to a secure login processing page. The NetID and password are not stored on the login page of the My UW-Madison portal but are passed through to the secure page and authentication is checked to ensure the NetID and password are valid. Additional information about My UW-Madison, including its security, can be found by clicking the 'About My UW-Madison' link on the login page. The largest threat to the security of the data comes from users voluntarily sharing their NetID and password with others.

Will my Social Security Number or Individual Tax Payor Identification Number be displayed on these forms?
No. In order to increase the security of sensitive information, any identification number issued by the U.S. Federal Government has been removed.  Therefore, if you use a printout of this form and submit it for tax filing purposes, you must write your number in the field provided on either the W-2 or 1042-S.

Are you using a Macintosh computer and cannot view your form(s)?
Safari on the Macintosh will automatically open the statement however if you are using  Firefox, click on the link for the tax statement and then a dialog box will open, select open with "Preview" then click okay.

Who should I contact if I have a question regarding the information on my tax statements?
If the question pertains to the contents or existence of a tax statement, contact your payroll office.

How can I request alternate formats of my tax statements?
Original, paper copies of year-end tax statements produced by the Office of Human Resources will continue to be disseminated by first-class U.S. Mail in compliance with the governmental regulatory provisions.
Duplicate statements can be obtained through the normal duplicate statement request processes; by calling (608) 262-5931 or you can request one by using this online form: www.bussvc.wisc.edu/ecbs/uw1180ee.html.

Does the Freedom of Information Act apply to information in the My UW-Madison portal?
Data associated with your individual employee record is accessible only by you. The same standards for the Freedom of Information Act that apply to data printed on paper also apply to data in the My UW-Madison portal.

How do I print my tax statements?
If you choose to print your statements, these instructions apply to the Internet Explorer browser. If you are using a different browser, the instructions may be different.

Where can additional information be obtained?
Federal (IRS) forms and publications may be obtained by visiting www.irs.gov or by calling (800) 829-3676. Individual tax advice may be obtained by calling (800) 829-1040.  State forms, publications and individual advice may be obtained by calling (608) 266-2486 or (608) 266-2772.  The state Department of Revenue web address is www.dor.state.wi.us.

Could I receive other non-Office of Human Resources year-end tax forms?
Yes. There are other year-end tax forms produced outside of the Office of Human Resources.  Examples of these forms include the following: 1098-T (tuition), 1098-E (interest on student loans), 1042-S and 1099-MISC (the latter both relating to accounts payable payments).
