Beneficiary Designation

Standard Sequence by Plan

Each of your benefit plans has a default or "standard sequence" of beneficiaries to which survivor benefits will be paid if you do not file a designation form naming specific beneficiaries, or if all of your named beneficiaries are deceased. The default payment sequences vary by benefit plan and each benefit plan has a separate Beneficiary Designation form.

Whenever you have a life event change it's important to review your beneficiary designations and complete and file a new Beneficiary form if necessary.

  • You can name specific beneficiaries for your regular retirement account.
  • You can name specific beneficiaries for your voluntary additional contributions, if any.
  • You can name specific beneficiaries for your tax sheltered contributions, if any.

    Beneficiary Designation form ET-2320 is used for the above process. Or, you can request an Alternate Beneficiary form from Employee Trust Funds if ET-2320 does not meet your needs.

    Note: Beneficiaries named on either form apply to both your Wisconsin Retirement account and your State Group Life Insurance proceeds unless you complete and file separate forms for each program. Access the Beneficiary Form.

State Group Life

Employee Coverage

  • You can name specific beneficiaries for your State Group Life Insurance, including all levels of coverage.
  • You can name separate beneficiaries for your basic, and/or supplemental and/or additional life insurance.

    Beneficiary Designation form ET-2320 is used for the above process. Or, you can request an Alternate Beneficiary form from Employee Trust Funds if ET-2320 does not meet your needs.
