- Employee Campus Information, UW1270
- W-4 Form–Employee’s Withholding Allowance Certificate, UW1389-W4
- I-9 Employment Eligibility Verification
I-9 Section 1 must be completed by the first day of employment.
Section 2 must be completed by the third day of employment.

To have health coverage effective on the first of the month after your date of hire, your appointment must begin on or before that date and you must file your health plan application on or before that date.

Timely Submission of Your Benefit Plan Applications - Do Not Delay!
The timely submission of your benefit applications is necessary if you want to take full advantage of your benefit package. Submit your applications within 30 days after the start date of your appointment. Coverage is typically effective on the first of the month, after the applications are received; some plans have additional requirements.
