- Athletics, Law School, Regents, Military, SSDI, and Midwest Poultry Remission Form.
Fellowship Forms
- WDGF and FLAS Remission Form.
- AOF Remission Form (for GRS directors only).
- For fellowships administered by the Graduate School, programs do not need to submit a remission form; these are handled by the Graduate School.
- For fellowships NOT administered by the Graduate School, consult the tuition remission policy. If the fellowship qualifies for remission, fill out this form and send via email attachment to offr@grad.wisc.edu.
- For NIH/NRSA fellowships, submit a Third Party Deferral Form to the Bursar's Office
