• RA, TA, PA, LSA, and Instructional Staff Remission Form.
• Athletics, Law School, Regents, Military, SSDI, and Midwest Poultry Remission Form.

Fellowship Forms

• WDGF and FLAS Remission Form.
• AOF Remission Form (for GRS directors only).
• For fellowships administered by the Graduate School, programs do not need to submit a remission form; these are handled by the Graduate School.
• For fellowships NOT administered by the Graduate School, consult the tuition remission policy. If the fellowship qualifies for remission, fill out this form and send via email attachment to offr@grad.wisc.edu.
• For NIH/NRSA fellowships, submit a Third Party Deferral Form to the Bursar's Office
