Valid IRS Naming Convention
Enter the name of the student (preferably surname first) whose Taxpayer Identification Number was provided in positions 2 to 10. Left justify and fill unused positions with blanks or spaces. A comma (,) between the last name and first name is allowable, but not necessary. A period (.) after the middle initial is not necessary. Surname prefixes such as Van are considered part of the surname, and can be attached. Names with an apostrophe, such as O'Neill may be written with or without the apostrophe. Upper case format is preferred.
