Authorization for Wisconsin Air Services
- If you would like to use the contract for Wisconsin Air Services for charter air service, complete the form and fax to UW Travel Manager (608) 262-5060 for prior approval.
- Once approved the form will be faxed back to you.
- Forward the approved form to Wisconsin Air Services by faxing to Sue Seibel at (608) 246-4727.
- Form needs to be received by WAS prior to flight confirmation.
